Pilots struggled with flight systems in Indonesia crash

"The flight from Denpasar to Jakarta experienced stick shaker activation during the takeoff rotation and remained active throughout the flight", the report said.

Lemme said he was also troubled that there weren't easy checks to see if sensor information was correct, that the crew of the fatal flight apparently wasn't warned that similar problems had occurred on previous flights, and that the Lion Air jet wasn't fixed after those flights.

The report does not reach any conclusions.

The search for the cockpit voice recorder is proving hard after investigators said last week its "ping" signal was no longer being detected. "We are confident in the safety of the 737 MAX".

That software, which has been described as several lines of coding, was identified in the Boeing manual as the maneuvering characteristics augmentation system, or MCAS.

The investigators are focusing on whether faulty information from sensors led the plane's system to force the nose down.

The pilots were quickly locked in a deadly game of tug of war with a brand new Boeing 737 Max 8.

The Indonesian National Transportation Safety Committee has retrieved one of the plane's black boxes - the flight data recorder - but is yet to locate the cockpit voice recorder. The captain's stick shaker activated shortly after takeoff and remained nearly continuously active.

The pilot of the 28 October flight chose to press on to Jakarta after shutting down the plane's anti-stall system, Utomo said.

KNKT investigator Nurcahyo Utomo on Wednesday said the safety committee had not yet determined if the system was a contributing factor.

"It's all consistent with the hypothesis of this problem with the M.C.A.S. system", said R. John Hansman Jr., a professor of aeronautics and astronautics and director of the global centre for air transportation at the Massachusetts Institute of Technology. The government report suggests the airplane should have been removed from service because of earlier reports about problems. On Nov. 6, family members strewed flowers into the Java Sea, where the twin-engine jet had nose-dived into, in commemoration.

She says her son, Capt. Bhavye Suneja told her there was no updated training simulation session when Lion Air started using the new aircraft.

The MAX aircraft is the latest version of Boeing's popular 737 jetliner. More than 200 MAX jets have been delivered to airlines around the world.
